In order for me to evaluate group projects and team dynamics effectively, it is very important for individual team members to provide input and feedback on yourself and your fellow team members.

Please fill out this form for EACH person on your team, including yourself. It is sometimes revealing to see how others perceive your work, compared with how you see it. Your feedback will remain confidential.

This category rates how well the individual demonstrated that they can do their part of the project, and at the same time provide timely assistance to other team members who need help, while also asking for backup if their part of the project became overwhelming.
